Our client a high profile NHS organisation is seeking to appoint an Interim Discharge Co-ordinator. This role will be at interim contractor rates under the CCS NMNC Framework at Band 3.

This role will start ASAP.

Main Responsibilities

  • To work as part of a responsive, flexible team supporting the planning, facilitation and delivery of effective patient flow in the tasks associated with daily simple and planned complex discharges within all ward areas. To proactively participate and support the ward-based Multi-Disciplinary Teams and Integrated Coordination Hub in safe, timely and effective discharge for all patients at Birmingham Community Healthcare Foundation Trust. The postholder will be expected to work flexibly to ensure that service provision is maintained and across all areas.
  • To support a comprehensive and responsive service to all wards/areas in relation to all aspects of discharge planning and facilitation, including the completion and submission of referrals to the Integrated Coordination Hub including formal Notifications.  
  • To actively liaise with patients and/or their relatives ensuring they are involved and informed regarding all aspects of their discharge from the acute setting or transfer to alternative settings as part of their discharge pathway.
  • To ensure that the Nurse in Charge and MDT is updated on progress or changes in discharge plans for any patient including appropriate escalation where relevant.
  • To ensure any changes related to a patient’s discharge planning, are communicated in a timely manner to the Integrated Coordination Hub.
  • To maintain ward-level records related to the discharge for all patients and actively contribute to other systems where relevant.
  • To support the process for clinical decision making as part of the D2A (Discharge to Assess) pathways alongside the Integrated Coordination Hub. 
  • To process all non-emergency patient transport requests.
  • To record all requests for internal bed transfers and ensure the Integrated Coordination Hub is aware of these.
  • To liaise with wards ensuring a timely response is provided for all aspects of support with discharge planning, referring to the appropriate teams as required;
  • Communicate effectively and appropriately, demonstrating professional behaviour always;
  • To ensure accurate reporting of any patient who is recorded as Medically Fit for Discharge or well enough to transfer to another setting as part of their discharge pathway, to the Integrated Coordination Hub.
  • Attend the Daily Operations (Ops) meetings as required to provide timely and regular updates with progress against discharge for patients the postholder is involved with.
  • To ensure accurate reporting related to simple discharge to the ward nurse in charge and the Integrated Coordination Hub as required;
  • To develop a good relationship with ward staff, patients, relatives and other members of the team as required;
  • Contribute to team discussions as required, making suggestions for service improvements to both the Clinical Team Lead, MDT, Matron and Integrated Coordination Hub Service Lead
  • To contribute to the record of care using an appropriate standard of documentation on IT systems and related paper records;
  • To provide comprehensive verbal reporting to the appropriate MDT ward-based staff, the Clinical Team Leader, and Integrated Coordination Hub;
  • To maintain a high level of administrative organisation and documentation to provide a consistent level of information when not on duty.
  • To actively participate in discussions and provision of information specifically related to those patients who are deemed Medically Fit for Discharge or whose discharge is delayed for any reason.
