Senior CHC Nurse Assessor

Finegreen are supporting an NHS organisation in the West Midlands who are looking to appoint a Senior CHC Nurse Assessor.

The role will be hybrid working, on an interim basis of 6 months, with a competitive rate dependant on experience.

The post holder will support in Continuing Healthcare assessments, reviewing and monitoring care of patients with assistance from senior professionals.

Key responsibilities will include:

  • Responsible for complex clinical caseload, undertaking reviews;

  • Support in the delivery and implementation of DOLS/LPS for your defined caseload;

  • Support the clinical team in maintaining data systems in line with standard operating policy and procedures;

  • Providing specialist advice and guidance on functional and information matters relating to the service;

  • Assist with CHC assessment and decision-making processes;

  • Work with health, social care, and independent sectors professionals.

The ideal candidate will have:

  • Educated to degree level or equivalent level of experience working in a specialist area;

  • Dual registered LD Nurse/Registered Nurse with a current NMC registration or equivalent;

  • Previous experience of undertaking Continuing Care Assessments;

  • Ability to write reports and review by senior members of the team;

  • Excellent communications skills and liaising with external agencies.
