Finegreen are supporting an NHS organisation in the West Midlands who are looking to appoint a Senior CHC Nurse Assessor.
The role will be hybrid working, on an interim basis of 6 months, with a competitive rate dependant on experience.
The post holder will support in Continuing Healthcare assessments, reviewing and monitoring care of patients with assistance from senior professionals.
Key responsibilities will include:
Responsible for complex clinical caseload, undertaking reviews;
Support in the delivery and implementation of DOLS/LPS for your defined caseload;
Support the clinical team in maintaining data systems in line with standard operating policy and procedures;
Providing specialist advice and guidance on functional and information matters relating to the service;
Assist with CHC assessment and decision-making processes;
Work with health, social care, and independent sectors professionals.
The ideal candidate will have:
Educated to degree level or equivalent level of experience working in a specialist area;
Dual registered LD Nurse/Registered Nurse with a current NMC registration or equivalent;
Previous experience of undertaking Continuing Care Assessments;
Ability to write reports and review by senior members of the team;
Excellent communications skills and liaising with external agencies.
