Local Care Programme Delivery Manager

Job Description

Finegreen are working with an NHS organisation in London who are looking to appoint a Local Care Programme Delivery Manager. This role will be for 3 months with a competitive rate based on experience.

The post holder will be responsible for providing professional expertise to the outcome of the business processes for the Department or Directorate.

Key responsibilities will include:

  • Define current processes, facilitate discussions, and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery;

  • Developing high quality, at scale Primary Care, through Primary Care Networks (PCN’s);

  • Developing a framework for the delivery of borough based Integrated Care Partnerships, and facilitating the practical integration of Primary, Community, Community Mental Health, Social Care and Voluntary Care, around PCN’s;

  • Working with and through local people and organisations;

  • Improving access, experience, and outcomes for locality;

  • Developing consistent, high quality service speculations across location, to drive and deliver an equality of access to healthcare;

  • Integrated work with Local Authority at a locality level, to jointly design and develop care approaches;

  • Ensuring high quality and consistent support to Care Homes;

  • Hold a clinical background, ideally still registered to appropriate body;

  • Commit to working and engaging constructively with internal and external stakeholders on a range of contentious issues;

  • Responsible for supporting the commissioning of projects and procurement of services to support project delivery;

  • Highlight exceptions and risks ensuring mitigating action can be taken to keep the programme on track;

  • Responsible for proposing and drafting changes, implementation and interpretation to policies, guidelines, and service level agreements (SLA’s) which may impact within Department or Directorate.

The ideal candidate will have:

  • Extensive knowledge of specialist areas;

  • Knowledge of contract management–both with providers and patient engagement;

  • Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service delivery;

  • Evidence of planning and delivering programmes and projects and services on time;

  • Team working skills;

  • Self-motivated.
